Med-QUEST (Hawaii Medicaid)

Med-QUEST is Hawaii's Medicaid program, called QUEST Integration. It gives free or low-cost health coverage to children, pregnant individuals, parents, seniors, people with disabilities, and low-income adults. Hawaii expanded Medicaid, so low-income adults without children can qualify too. The fastest way to apply is online through the KOLEA portal at medical.mybenefits.hawaii.gov, or call Med-QUEST Customer Service at 1-800-316-8005. TTY users call 1-855-889-4325 or 711.
There are four ways in. You can apply online, by phone, by mail, or by fax. To apply by mail or fax, fill out form DHS 1100, Application for Health Coverage and Help Paying Costs, and send it to the Med-QUEST Eligibility Office nearest you.
QUEST Integration covers several groups, each with its own path: adults age 19 through 64, children under 19, pregnant individuals, parents or caretaker relatives living with a child, and people who are aged, blind, or disabled. There are also Medicare Savings Programs and a Medically Needy spenddown program for people with high medical bills.
New parents get a helpful rule. If you are enrolled in a QUEST Integration health plan when you deliver, your newborn is automatically enrolled in your plan for at least 30 days from birth. If your eligibility ends, you can reapply at any time.
A big change is coming, so plan for it now. Under federal law HR1, signed July 4, 2025, starting January 1, 2027 many adults in the expansion group face two new rules: eligibility renewals every six months instead of once a year, and a work or community-engagement requirement.
The new requirement asks affected adults to show they work or volunteer at least 80 hours a month, or earn at least $580 a month, or are enrolled at least half-time in school. Med-QUEST advises members to set up online account access and save pay stubs now. After you apply, an eligible person gets an enrollment packet by mail to pick a health plan.
